*Additional surcharge for VIP memberships will be applied for patients who present with periodontal disease due to the additional services and time required to treat this condition.

Terms & conditions
Studio Dental membership fee is due at enrollment, and is valid for one year from the time of enrollment. VIP Membership is non-refundable and non-transferable. To receive the $500 credit for orthodontic treatment, the member must be enrolled in the VIP Membership plan for the entire duration of the treatment.
